Tyler Oral and Facial Surgery center and it’s adjoining outpatient surgery facility, Turtle Creek Surgery Center, represent the realization of a dream—to provide our patients with the latest medical technology while providing thoughtful, individualized care. We offer consultation offices and surgery suites all in one location, employing many of the same fail-safe systems available in most advanced hospital settings.

Turtle Creek Surgery Center is a Texas Department of Health Certified Facility designed for more advanced surgical and anesthesia needs. Procedures such as facial cosmetic surgery, TMJ surgery and advanced dental implant surgery can be accomplished safely in our offices. We are one of the few Oral and Maxillofacial Surgery practices in the nation to provide such services. And with four surgeons at our facility five days a week, it means optimum convenience for our patients. We can typically schedule a consultation within one week and emergencies can be handled quickly and efficiently

Despite these advanced features, our fees are comparable (and in many cases less costly) when compared to similar practices in the region. Each doctor has a qualified staff who works with your insurance company and we also offer 0% interest, flexible financing plans to those who qualify. Our Doctors

Mak Saigusa, DMD, MD, FACS

Mak Saigusa brings a unique level of expertise to the practice of oral and maxillofacial surgery in East Texas. Experienced in all areas of oral and facial surgery, Dr. Saigusa is particularly interested in facial reconstruction due to traumatic injuries and congenital/developmental deformities. He also focuses on the surgical treatment of TMJ conditions and dental implants.

Born in Tokyo, Japan, Dr. Saigusa came to the United States while still a teenager, finishing his senior year of high school as an exchange student in the small farming community of Markesan, Wisconsin. He received his Bachelor of Science degree from Samford University in Birmingham , Alabama, and earned a D.M.D. from the University of Alabama School of Dentistry. In 1991, he moved to Texas to attend medical school at the University of Texas Health Science Center in San Antonio.

After receiving his M.D. and completing his residency in Oral and Maxillofacial Surgery, he moved to Tyler in 1997 to join the practice of Dr. James Holton.

Dr.Saigusa resides in Tyler with his wife, Jill, and two sons. He is involved in a number of charitable causes including Texas Missions of Mercy, which each year provides free medical and dental services to thousands of East Texans.

Jayson Terres, DDS, MD, FACS, FAACS

Dr. Jayson Terres joined Tyler Oral and Facial Surgery in the summer of 2005. He was particularly drawn to the Tyler area and the practice because of the extensive medical community and the progressive treatments it offers. Dr. Terres brings another level of expertise to the Tyler Oral and Facial Surgery team, led by Drs. James Holton and Mak Saigusa. Trained in all aspects of facial surgery, his interests lie particularly in facial cosmetic and reconstructive surgery as well as facial trauma surgery. Because of the scope of his knowledge and his extensive oral surgery and medical training, Dr. Terres also performs a full spectrum of facial rejuvenation procedures thru surgical techniques including, endoscopic brow and forehead lifts, eyelid surgery, facelift, chin augmentation, liposuction, and also through non-surgical procedures such as Restylane, Botox, chemical peels and skin care.

A native of Michigan, he attended The University of Michigan in Ann Arbor where he received a Bachelor of Science degree in Neurobiology. Following his undergraduate degree, he attended Columbia University College of Physicians and Surgeons, School of Dental and Oral Surgery, in New York City and earned a D.D.S. After his dental surgery training, he moved to San Antonio, Texas where he earned his M.D., at the University of Texas Health Science Center, San Antonio. Following this, he completed an internship in General Surgery and then a six-year residency program in Oral and Maxillofacial Surgery.

Dr. Terres serves as a Clinical Associate Professor for the UT Health Science Center in San Antonio as well as a resident advisor for the United States Military Army residency programs. He has lectured at the national level for Cranial Reconstruction and continues to seek the latest and most patient-friendly surgical technologies, advancements and procedures.

Beyond the operating room, Dr. Terres values time with his wife Veronica and his son, Jayden. Part of the philosophy that drives Dr. Terres’ success behind being a family man and a surgeon is maintaining a sense of humility and understanding that both take an exorbitant amount of hard work, dedication, responsibility and empathy. He finds his involvement with Tejas Missions, a ministry providing volunteer medical and dental treatment to underdeveloped regions within Central America very rewarding and humbling. He deems it a responsibility, as a physician to help people in need, especially when an opportunity presents itself to offer surgical services for complex reconstructive cases.

Kourt Chatelain, DMD

Dr. Kourt Chatelain is from Ogden, Utah. He graduated from Weber State University with a Bachelor of Integrated Sciences. He then entered The University Nevada Las Vegas where he received doctorate training and graduated with honors as a Doctor of Dental Medicine in 2009. Upon completion of education at UNLV, Dr. Chatelain completed his training in oral and maxillofacial surgery at Loma Linda University. Dr. Chatelain is trained in multiple surgical and medical disciplines, but his specialty is all facets of Oral and Maxillofacial Surgery, including Craniofacial Surgery, Facial Reconstructive Surgery, Temporomandibular Joint Arthroscopic/Laser Surgery, Distraction Osteogenesis, Orthognathic Surgery, Sleep Apnea Surgery and Implant Surgery.

Dr. Chatelain also enjoys serving others in humanitarian efforts, volunteering his medical care in underdeveloped countries including Bangladesh, Zambia, and Peru. Dr. Chatelain is well-trained in full-scope Oral and Maxillofacial Surgery with an increased interest and experience in advanced reconstructive bone grafting, dental implants, Orthognathic Surgery, trauma and wisdom teeth removal.

Outside the office, Dr. Chatelain enjoys spending time with his wife Jamie and their four children. He also enjoys classic car restoration, canyoneering, and water activities.
